Requirements
- Associate's or Bachelor's degree in Nursing, Social Work, Public Health, Health Administration, or a related healthcare field OR Certificate/Diploma in Practical Nursing (LVN/LPN).
- Active RN, LSW/LMSW/LCSW, CCM/ACM, CHW, LPN/LVN, or other relevant clinical or care management credential preferred.
- 2+ years of experience in care navigation, patient advocacy, care management, case management, home health, discharge planning, utilization management, or clinical social work, preferably supporting older adults or Medi…
- Solid knowledge of Medicare rules, eligibility, benefit structures, claims, and appeals.
- Deep understanding of Social Determinants of Health (SDOH) and experience working with diverse and underserved populations.
- Empathy, patience, and strong communication skills— you can explain complex things simply.
- Comfort working with digital tools, EMRs / care-management software, and adapting to new tech.
- Commitment to continuous improvement— you’ll raise issues, suggest solutions, and help refine Baba’s model.
